Duties and Responsibilities:  

Reporting to the Supervisor, Housing Support and/or Coordinator, Housing Administration and Development or designate, this position will be responsible for: 

  • Providing short (up to 30 days) to long term (36+ months) on-site and in-home intensive case management support to individuals and families who may be vulnerable, at risk or experiencing homelessness; 

  • Creating and updating case plans on a regular basis;  

  • Providing crisis intervention and clinical counselling using various therapeutic modalities; 

  • Assessing bio-psychosocial needs and makes referrals to appropriate community resources;  

  • Completing suicide, homicide and other risk assessments, and where appropriate, co-creating safety plans with clients;  

  • Completing applications to the Justice of the Peace for Form 2 - Order for Examination under s.16 of the Mental Health Act, where appropriate; 

  • Coordinating and facilitating interdisciplinary case conferences for high acute individuals with complex needs with internal and external sector stakeholders in health, justice, housing, homelessness, social services and others, as necessary; 

  • Facilitating client, family, resident education and support groups, as needed;  

  • Scheduling and facilitating weekly conference calls with community agencies for emergency shelter diversion or other purposes, as directed;  

  • Participating in community tables / meetings, as assigned;  

  • Reviewing medical and other documentation to support case plans;  

  • Attending encampments and/or responding to outdoor homelessness, as directed;  

  • Providing on-site task supervision and facilitates training for social work students (BSW and MSW);  

  • Liaising, providing consultation and acting as a resource to management, internal and external staff and community agencies to ensure client and departmental needs are met;  

  • Maintaining professional relations with clients, the public, elected officials, colleagues, internal and external agency staff 

  • Maintaining and documenting all client interactions using a CRMS systems and the Homeless Individuals and Families Information System (HIFIS), social assistance electronic data management systems or other electronic tools, as directed;  

  • Composing professional correspondence, as required;  

  • Working in compliance with the Code of Ethics and Standards of Practice under the Ontario College of Social Workers and Social Service Workers and other relevant program guidelines, policies, procedures and legislation;  

  • Will perform Occupational Health & Safety duties as outlined in the Corporation’s Health and Safety Program and as required in various community settings; 

  • Will perform other related duties as required. 

Qualifications:  

  • Must have a four (4) year, post-secondary school University Degree in Social Work or Ontario Ministry of Education equivalencies; 

  • Must have over one (1) year of work experience in Social Work, Social Service, Health, or Justice fields;  

  • Must be registered and maintain registration in good standing with the Ontario College of Social Workers and Social Service Workers as a condition of continued employment in this position; 

  • Must have the ability to travel to offsite locations in a timely and expedient manner as required. If method of travel is by vehicle, a current, valid and lawful Driver’s Licence will be required in accordance with the Highway Traffic Act and must provide a driver’s abstract as a condition of employment; 

  • Must have experience in a computerized office environment utilizing the Microsoft Suite of Products (such as Word, Outlook, Excel, etc); 

  • Training and use of the Homelessness Individuals and Families Information System (HIFIS) and CRMS Software is considered an asset; 

  • Must be a mature person who can demonstrate sensitivity to client needs and who has a good knowledge of social service, homelessness and housing programs and related legislation; 

  • Must have exceptional interpersonal, communication and organizational skills; 

  • Should have knowledge of all applicable Acts and Regulations; 

  • Proven commitment to ongoing professional development will be considered an asset; 

  • Related community and volunteer experience is a definite asset. 

Physical Demands: 

  • Given that this job is a new position a Physical Demands analysis will be completed with the incumbent after approximately six month of occupying the position.
